The going theory is that they were giving him aspirin and when he told them to leave him be that also stopped, removing the thing making him worse.

I think, their question is due to the semantics of the caption, as it can also be understood as “taking somebody off asprin makes haemophilia worse”.
Although it is a little ambiguous, I read it correctly the first time.
Me too. It just appeared to me after thinking, why the OC came up with that comment.
I also had to do a double take. It was phrased a bit oddly.
Which was likely not because Rasputin understood the connection, he was just a talented grifter that got lucky.
